Do You Have A Plan?

We plan for trips, holidays and birthdays, but often ignore a plan for one of the most impactful stages of our lives. Because every life will end, one of the most important conversations we can have is about our plans for the care we want to receive on our end of life journey. Often this conversation is with a healthcare provider and close family.

Our Palliative Care team has this conversation 60-75 times a week with patients and families when they make house calls. Our Nurse Practitioners travel to visit with individuals and families, in the privacy of their homes, to discuss personal goals of care.

Palliative Care is a resource for anyone living with a serious illness such as heart failure, COPD, Cancer, Alzheimer’s, Parkinson’s disease, and many others. Palliative Care can be provided along with curative treatment and does not depend on prognosis.

Nurse Practitioners ask how our healthcare team members can assist in meeting needs. What are the goals of care for the day, week or at the time of our visit?

Palliative Care Resources and Hospice Home Care offer great support and a variety of services for what can be a beautiful, peaceful journey, but we first must ‘break the ice’ around the uncomfortable, inevitable subject; end of life.

Facing the end of life can be hard. Facing it alone, in the absence of family, friends, financial resources and qualified caregivers, is even harder.

The Tribute of Life Foundation believes that everyone deserves to complete their journey in a warm, clean bed, surrounded by people who care deeply about meeting their medical, social, emotional and spiritual needs. Through our 'No One is Alone' initiative, we are committed to funding two beds in the Comfort Care Center for people who could not afford it otherwise.
