Palliative Community Resources

Palliative Community Resources

Palliative Care is an in-home medical resource for anyone living with a serious illness, seeking to improve their quality of life through improved symptom management, and understanding the choices for treatment. The services available through palliative care may be helpful to anyone experiencing discomfort and disability late in life. Palliative Care can be provided along with curative treatment and home health, and does not depend on the likely outcome of a disease process.

Benefits of Palliative Care

  • Patients may maintain treatment that might cure a serious illness
  • Palliative Care may transition to a higher level of care if the doctor and Palliative Team believes ongoing treatment is no longer helping.
  • Palliative visits by a Nurse Practitioner happen wherever you call home. No need to drive to an appointment.
  • Patients may continue receiving Home Health and Palliative Care at the same time.
  • A Nurse Practitioner will support your care during transitions or when you are unable to go to a regular physician.

Palliative Care FAQs

Hospice services are covered by Medicare or Medicaid, private insurance, private pay or through charitable care. This care is provided to all in need of our services regardless of age, sex, race, creed, marital status, sexual orientation, color, national origin, religion, citizenship status, veteran status, illness, handicap, disability or ability to pay.

Hospice does not mean giving up hope! It is possible that a patient will improve, resulting in discharge from hospice services. Or the patient or caregiver might decide to stop hospice care to seek aggressive, curative treatment. Hospice services can easily be resumed, if needed, at a later time.
